When an insured patient is admitted, send the request to the insurer or TPA with the member and policy numbers, the diagnosis and the estimated cost, picked straight from the bed. Record each answer as it comes: the insurer's question and your reply, the approved amount with its approval number, more asked when the stay runs longer, then the final bill. What the bill is above the approval is the patient's to pay, and when the insurer settles, what it did not pay shows as disallowed. The open requests are listed longest wait first, with those waiting over a day for the insurer picked out, and each insurer's approved, billed, settled and disallowed amounts.
